Overview

Basic Support Services under the 245D Waiver are designed to support individuals with disabilities and related needs in maintaining independence, safety, and meaningful participation in their communities. These services focus on assisting individuals with essential daily living activities while respecting personal choice, dignity, and individual goals.

Basic Support Services are person-centered, meaning all support is delivered according to an individual’s assessed needs and documented service plan. Services are flexible and may be provided in the individual’s home, community, or other approved settings, depending on eligibility and authorization.

The goal of Basic Support Services is not to replace independence, but to strengthen it by offering structured, consistent support that allows individuals to live as independently as possible while remaining safe and supported.

24-Hour Emergency Assistance

The primary purpose of this service is to protect individuals during unforeseen situations that require immediate attention. Emergencies may occur at any time, and access to trained support staff can be critical in preventing further risk.

24-Hour Emergency Assistance is a Basic Support Service under the Minnesota 245D Waiver that provides immediate, short-term support during unexpected situations that place an individual’s health, safety, or stability at risk. This service is designed to respond to urgent, unplanned events that cannot be delayed and require timely intervention to prevent harm, crisis escalation, or disruption of essential daily living.

Emergency assistance is not intended to replace ongoing services or medical care. Instead, it offers temporary, responsive support to help stabilize a situation and ensure the individual remains safe until the emergency is resolved or appropriate long-term supports are in place.

Homemaker Services

Homemaker services are designed to support individuals who need help managing everyday household tasks so they can continue living safely, comfortably, and independently in their own homes. These services are ideal for older adults, individuals with disabilities, or anyone recovering from illness or surgery who may find daily home activities physically challenging or overwhelming. By providing reliable, respectful assistance, homemaker services help create a clean, organized, and nurturing home environment that promotes overall well-being.

Our homemaker support goes beyond basic housekeeping. Caregivers understand that a home is deeply personal, and every individual has unique routines, preferences, and cultural values. Services are delivered with dignity and attention to detail, ensuring that each person feels respected and in control of their living space. Individual Community Living Support (ICLS)

Individual Community Living Support (ICLS) is a person-centered service designed to help adults with disabilities live independently, actively, and safely within their own homes and communities. This service focuses on building essential life skills, strengthening confidence, and promoting meaningful community participation rather than providing hands-on care alone. ICLS empowers individuals to take control of their daily lives while receiving the right level of guidance and support based on their unique goals and abilities.

ICLS services are highly individualized. Each person has different strengths, challenges, and aspirations, and support plans are developed to reflect those differences. Caregivers work collaboratively with individuals to identify goals such as managing a household, developing social skills, improving time management, or increasing independence in community settings. The emphasis is always on teaching, coaching, and encouraging—helping individuals learn how to do things for themselves, not doing everything for them.

 

Individualized Home Supports (IHS)

Individualized Home Supports (IHS) is a flexible, person-centered service designed to help individuals with disabilities live safely and independently in their own homes. This service is tailored to each person’s unique needs, goals, routines, and preferences, ensuring support is provided in a way that respects personal choice and promotes independence. Individualized Home Supports can be delivered in a person’s own home or in a shared living arrangement, depending on what best supports their lifestyle.

The core focus of IHS is individualized care planning. Support plans are developed collaboratively with the individual and, when appropriate, their family or support team. These plans identify specific areas where assistance is needed—such as daily living activities, health and safety support, or skill development—while also highlighting strengths and long-term goals. Services are adjusted over time as needs change, ensuring continuous, appropriate support.

 

Night Supervision

Night supervision provides overnight support to ensure individuals remain safe, comfortable, and secure during evening and nighttime hours. This service is ideal for individuals who may need assistance during the night due to medical needs, safety concerns, or anxiety when alone. A trained support staff member is present or on call to monitor well-being, respond to needs, and provide reassurance throughout the night.

Night supervision helps prevent risks such as falls, wandering, or missed medications, while also offering peace of mind to individuals and their families. Support is delivered respectfully and with minimal disruption, allowing individuals to rest comfortably knowing help is available if needed.

Respite Care (In-Home or Out-of-Home)

Respite care provides temporary, short-term support for individuals while giving family members and primary caregivers a much-needed break. This service can be provided in the individual’s own home or in an out-of-home setting, depending on the level of care needed and family preferences. Respite care ensures continuity of care while maintaining comfort, safety, and familiar routines.

In-home respite allows individuals to remain in a familiar environment while a trained caregiver provides supervision, personal care, and daily living support. Out-of-home respite offers care in a safe, supportive setting and can include structured activities, social interaction, and 24-hour supervision if required. Both options are designed to reduce caregiver stress while ensuring the individual receives quality, compassionate care.
